Special Notice Regarding Medicare Reimbursement


Merlexi Craft™ is Medicare coded, but is not a Medicare provider. Therefore, as a medical professional, you will need to make a statement of medical necessity on DMERC form 02.03B if you are recommending that a patient receive a wheelchair. Please bear in mind that Medicare reimbursement for K0004 and K0005 chairs require that the mobility impaired have a need for a wheelchair for home use (their mobility needs outside of the home are not considered by Medicare). For that reason, it is important that you focus on the home needs of the patient in the Medicare documentation, namely:

  1. Patient cannot navigate with canes, crutches or walkers and needs and wants to use a wheelchair more than 2 hours a day for activities of daily living.
  2. Individual cannot manage the heavier chairs because of physical limitations.
